INDICATIONS

Zenrelia is a prescription medication used to control itching and inflammation associated with skin allergies for dogs over 12 months of age.

WARNING: VACCINE-INDUCED DISEASE AND INADEQUATE IMMUNE RESPONSE TO VACCINES. Based on results of the vaccine response study, dogs receiving Zenrelia are at risk of fatal vaccine-induced disease from modified live virus vaccines and inadequate immune response to any vaccine. Discontinue Zenrelia for at least 28 days to 3 months prior to vaccination and withhold Zenrelia for at least 28 days after vaccination. Dogs should be up to date on vaccinations prior to starting Zenrelia. Do not use in dogs less than 12 months old or dogs with a serious infection. Dogs should be monitored for the development of infections because Zenrelia may increase the chances of developing an infection. Neoplastic conditions (benign and malignant) were observed during clinical studies. The most common side effects were vomiting, diarrhea and tiredness. Zenrelia has not been tested in dogs used for breeding, pregnant, or lactating dogs and has not been evaluated in combination with glucocorticoids, cyclosporine, or other immune suppressive drugs.

To redeem your rebate, an original licensed veterinary invoice showing product(s) purchased from your veterinary clinic is required. Clinic information, product purchase(s) and date of purchase(s) must be clear and legible. Elanco retains the right to request additional information when processing a rebate submission request. All rebate claims are subject to Elanco’s ﬁnal review and approval. Non-compliant materials will not be honored or returned. Offer not valid for product(s) purchased through retailers.

Offer valid only in US and US Territories. Void where prohibited or restricted by law. All federal, state and local laws and regulations apply. Fraudulent submission of multiple requests and/or providing false information disqualifies any rebate request and could result in federal prosecution under mail fraud statutes (Title 18, USC Sections 1341 & 1342) as well as applicable state laws.

For mail-in submissions, send the completed rebate form (name, address and offer code) and a copy of your veterinary invoice to: Elanco Rebates PO Box 1080 ATTN: (insert Offer Code) Grand Rapids, MN 55745-1080. For rebate questions, call 877-246-7654. Rebates may be issued in the form of a check or Elanco Prepaid Mastercard®. Rebates issued on an Elanco Prepaid Mastercard® can be redeemed as a virtual or physical card. Your payment will be delivered via email with instructions for claiming your prepaid Mastercard. You must claim your card within 3 months of the date these instructions are sent via email. Your right to the payment expires after that time. However, if a valid email address is not provided, a physical prepaid card will be sent to your mailing address on ﬁle. For rebate checks or physical prepaid cards, please allow 6-8 weeks for delivery. For virtual prepaid cards, please allow 2 weeks for rebate email delivery once the rebate is validated. Rebate checks are void 90 days after date of issuance and Elanco will not issue replacement checks.
